CONSULTING SUPPORT
Geriatric Care Management Carolyn Brennan-AlleyPersonalized support is available as needed, including 24 hours a day/7 days a week. We are experienced in crisis management and can assist when immediate needs abruptly change.

Ongoing monitoring of the elder’s functional status and the effectiveness of services assures that needs are being met with consideration of strengths, abilities, and existing resources and support services.

Among the many areas of support in which Brennan-Alley can assist is counseling, which can include issues of living arrangements, changes in physical and mental functioning, loss of family or friends, adjustments to retirement, planning for advance directives, powers of attorney, and end-of-life care.

Maine’s rural nature can make it difficult for clients to have personal needs met. We include services—such as church-going, customized outings, and travel escort—that address these cultural, spiritual, social, and community needs.

We can assist families with screening and hiring private caregivers, as well as manage long-distance coordination with other GCMs for clients in transition—both within the state and state-to-state—whether they are changing location for a summer, or a lifetime.
